I permit Centennial Baptist Church to take photographs and/or video of my child. I grant them full rights to use the images resulting from photography/video filming, and any adaptations of the images for their printed and online publicity, and social media.
As a parent/guardian of the child listed, I agree to uphold the rules and regulations of Centennial Baptist Church Children’s Ministry and will expect and require my child to abide by these guidelines.
I also understand and consent that should my child’s behavior be such that is deemed necessary (at the discretion of the Children’s Director and leaders of Centennial Baptist Church) for them to be sent home, I will pick them up upon notification.
I, being the parent or legal guardian of the child named above, do hereby consent to the participation of my child in the following activity, Emerald Crossing VBS, conducted by Centennial Baptist Church.
I certify that my child is physically fit and adequately prepared to participate in this event.
If my child requires any medications for medical conditions such as, inhalers, EpiPens, insulin, or seizures meds, I will supply it in a bag labeled with their name and instructions.
If my child has food allergies, I will supply their dinner/snack.
I will not hold Centennial Baptist Church, its officers, clergy, agents, employees, members, volunteers, or anyone acting on its behalf, responsible or liable for injury occurring to the named person in the course of the event.
I understand that I will be notified in the case of a medical emergency. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event that my child is injured or
becomes ill. Every attempt will be made to contact the parent/guardian or emergency contact listed. I authorize these persons, CBC Children's Director, or agent of CBC to act in my place to consent to all necessary and appropriate x-ray examinations, anesthetic, medical, or surgical diagnosis or treatment, and hospital care.
I understand that Centennial Baptist Church will not be responsible for medical expenses or legal obligations incurred solely on the basis of this authorization. I also understand that the designated adult chaperones reserve the right to
restrict my child from any activity that they do not feel is within the physical capabilities of my child.
Furthermore, I will not hold Centennial Baptist Church, the church staff, the event location, or any volunteer responsible for any injury that may occur to my child as a result of this event.
List any medications that may need to be administered while participating.
List any food allergies or other allergies.
